Twelve months into a post-ACL reconstruction, a client of mine had plateaued. His surgical team was happy with the structural outcome. His physio said the exercises were going well. But he was still limping. Still waking at 3am from aching. Still not back to playing the sport he’d been working towards for a year.

He came to see me after a friend mentioned our clinic. The problem with treating one layer

Most treatment models are built around a single modality. You go to physio and you get physio. You go to a massage therapist and you get massage. You go to a hyperbaric clinic and you get a chamber session.

Each of those things has genuine value. None of them, in isolation, addresses the full complexity of what happens when a body is injured, stressed, or carrying chronic pain.

Injury – and particularly prolonged pain – is never just about the original site. It creates compensation patterns in surrounding muscle groups. It provokes systemic inflammation. It disrupts sleep, which impairs the body’s own nocturnal repair processes. It often has a nervous system component that manifests as hypersensitivity, guarding, or a kind of whole-body low-level tension that no amount of targeted treatment at the injury site fully resolves.

Effective recovery requires addressing all of these layers simultaneously, or at least sequentially in a coordinated way. That’s what an integrated approach makes possible.

What each modality does – and why together is more than the sum of the parts

What Hydroxy Hyperbaric Therapy contributes

Hydroxy Hyperbaric Therapy works at the cellular level. Under gentle pressure, oxygen and molecular hydrogen dissolve into the blood plasma and reach tissues that inadequate circulation – or simply the normal limits of red blood cell delivery – can’t adequately supply.

For injured or inflamed tissue, this matters enormously. Inflammation creates a hypoxic (low-oxygen) micro-environment around the affected area. Cells in that environment can’t repair efficiently because repair is an aerobic process – it requires oxygen as the primary input for collagen synthesis, ATP production, and immune function. Hydroxy Hyperbaric Therapy overrides the limitation imposed by inflammation-driven hypoxia.

The molecular hydrogen component adds another dimension: selective antioxidant action at the mitochondrial level, which reduces the oxidative damage that prolonged inflammation produces in surrounding healthy tissue.

What musculoskeletal therapy contributes

Soft tissue work – cupping, dry needling, mobilisation, remedial massage – addresses what the chamber can’t: the mechanical and structural dimensions of pain and dysfunction.

Muscles that have been compensating for an injured joint develop trigger points, altered fascial tension, and movement patterns that persist long after the original injury has healed. These compensatory patterns become their own pain source. They restrict lymphatic drainage. They create secondary inflammation in tissues that were never involved in the original injury. And they don’t resolve by themselves, regardless of how well the primary site has healed.

Hands-on manual therapy mobilises these patterns. It improves circulation, stimulates lymphatic flow, breaks down adhesions, and restores range of motion. It also – and this is underappreciated – sends direct proprioceptive input to the nervous system, helping to re-educate the body’s movement and threat-response maps.

Why the combination is more effective than either alone

The mechanism is straightforward, even if it’s rarely explained this way: manual therapy mobilises tissue and improves local circulation, creating the conditions for cellular repair. Hydroxy Hyperbaric Therapy then floods that mobilised, circulation-improved tissue environment with oxygen and hydrogen – fuelling the repair at the cellular level that the manual work has set up.

Reverse the order and it works differently but equally well: the chamber session reduces systemic inflammation and improves tissue oxygenation, making the subsequent manual work more effective because you’re working with tissue that is less guarded, less hypoxic, and more responsive.

Either sequencing produces compounding effects. The whole genuinely is more than the sum of the parts.

What conditions benefit most from combined treatment

This integrated approach is particularly effective for:

  • Sports injuries – especially ligament and tendon injuries, which have poor blood supply and heal slowly even with good surgical or conservative management. The chamber addresses the oxygenation deficit; the manual work addresses the compensatory muscle patterns and restricted movement.

What this looks like in practice

The protocol varies by individual presentation. But a typical combined approach might look like:

  1. Musculoskeletal assessment – a proper clinical assessment to identify the structural picture: compensation patterns, range of motion deficits, tissue quality, neural tension.
  2. Initial manual treatment – addressing the most limiting tissue restrictions and trigger points, improving local circulation.
  3. Hydroxy Hyperbaric session – ideally within 24–48 hours of manual treatment, to flood the mobilised tissue environment with oxygen and hydrogen.
  4. Monitoring response – how the body responds across the first three to five sessions guides the subsequent protocol.
  5. Ongoing combined sessions – sequencing and frequency adjusted based on response. Some clients benefit most from alternating modalities in the same week; others respond better to manual treatment followed by a chamber course.

There’s no rigid template because bodies don’t respond rigidly. The assessment guides the sequencing.

The body psychotherapy dimension

Not everyone needs this dimension. But for clients who have tried the conventional physical pathway and continue to plateau, addressing the nervous system through somatic work alongside the physical modalities often breaks the cycle.

Frequently asked questions

Do I have to do both modalities or can I just do one?

You can absolutely start with just one. Many clients begin with either the chamber or musculoskeletal therapy and add the second once they understand their own response. That said, if your situation is complex or you’ve been managing something for a long time without resolution, starting with both from the outset typically produces faster results.

How many sessions of each will I need?

It depends on what you’re managing and how your body responds. Most combined protocols run over four to eight weeks, with a mix of two to four sessions per week across the two modalities. We assess your response after the first three to five sessions and adjust.

Is there an additional cost for combining modalities?

Each modality is priced separately. The value argument for combining them is clinical – faster outcomes mean fewer total sessions and better long-term results. We’ll discuss the protocol that makes sense for your situation and your budget.

Can I claim combined sessions on private health insurance?

Musculoskeletal therapy and remedial massage may be claimable under your extras policy depending on your fund. The Hydroxy Hyperbaric component is generally not currently rebatable for wellness purposes. We can provide appropriate documentation for the manual therapy component of combined sessions.
